You don't re-compile most of it.

If the machine is really old you'd probably be
better off re-compiling it all for the machine
with optimization turned on, debugging turned
off anyway in order to squeeze the best
performance out of it. Compiling the kernel
once and running "emerge --update --newuse --deep world"
on a Monday night and let it run to completion
by itself itsn't all that painful and would
give better results going forward than living
with the stock kernel and code that come
with most distros.
