Don't have one handy, but NoLo Press (IIRC) has a book full of sample 
tech-oriented legal documents like this.


That said, just wondering a few things I'd think would be fairly 
obvious:  a) why didn't you have the employee sign such a doc when they 
were hired?  and b) given that they didn't sign such a doc when they 
were hired, why should they agree to sign one now that they're leaving?
