I recommend Hewlett Packard, formerly "DEC Move". They will 
write up a contract to provide whatever services you need.

Typically, they provide a white-glove service, shutdown,
derack, pack, move, unpack, install in new rack, power up,
test, and guarantee everything will work or they'll fix it.

Not cheap, but if you want to maximize chances of success,
and have a budget, I can't say enough good things about
the few times I've used them.
